Pregnancy beyond 38 years old is considered a risk pregnancy. The main risks are

-for the mother:

-hypertension

-gestational diabetes

-thrombo-embolic accidents

-the use of cesarean, especially among primiparas

-mortality, 4 times higher at 40 than at 30 years old

-for the fetus:

-chromosomal abnormalities, including trisomy 21

-malformations

-mortality. We can distinguish,in the first quarter: spontaneous miscarriage,in the 2nd and 3rd quarters: fetal deaths in utero during childbirth: perinatal mortality

Therefore, any late pregnancy requires a targeted surveillance, both by the attending physician and the specialist member of the center where the delivery will be carried out

Monitoring will primarily allow for:

-clinical screening and ultrasound of fetal-maternal pathologies

-direction of prevention and in case of anomaly, the decision for early treatment or as a last resort, an IMG (Medical termination of pregnancy)

-Prediction of mode of delivery is best suited in case of full-term pregnancy
